VILLAGE PHARMACY IS RECEIVING DOSES OF COVID-19 VACCINE WEEKLY. YOU MAY SCHEDULE AN APPOINTMENT ON THIS WEBSITE. WE WILL CONFIRM YOUR APPOINTMENT AND MAY NEED TO REARRANGE TIMES IN ORDER TO PREVENT WASTE.
The vaccine is provided at no cost to you the patient. An administration fee will be billed to your insurance company or to the Health Resources and Services Administration (HRSA) if you do not have health insurance.

If you choose to receive the vaccine, fill out and submit the form below. Please bring your driver's license and insurance card or social security card to facilitate record keeping. After the injection, you should remain near for 15 minutes in the rare case of an allergic reaction. Covid Vaccine is available for anyone over 18 years of age.
